I have been in chastity 24/7 for almost two years now for My mistress, who is my wife and keyholder. Initially we started with plastic devices, and for more than a year I have worn a mature metal jailbird, usually with the spike-strip mounted to prevent escape (see picture) and add some CBT challenge.
Security is very important to my Mistress and me, but the jailbird is not 100% secure. Therefore we have been considering a PA piercing and invest in a new metal cage optimized for PA, such as Lori, Steelworx, or SteelwerksExtreme.
I am uncut and with all the cages I have used I have had problems with swelling of the foreskin from time to time, or problems with pinching of the rolled-up foreskin which is sometimes sticking out of the cage.
Therefore we have been considering for me to get a circumcision, although I have still some lingering doubt about it for fear of losing too much sensitivity, so I tend to postpone this decision, and would like to go ahead with the PA.
Now the questions I have are the following:
1. What would you advice as the sequence: First get a PA and then get a circumcision, or the other way around? What do you see as pros and cons of one scenario vs. the other?
2. Does anyone of you who are uncut have good experience with a PA piercing and wearing a chastity cage?

For the first question, I'd see a piercer who is a member of the APP for professional advice. Get your specific anatomy evaluated. Perhaps BME or Tribalectic might have advice. I might check my copy of the Piercing Bible to see if there's info there.
You might be able to be pierced and heal with a curved barbell before or without circumcision, as long as the ball end is small enough. Specifically, I'm wondering if a system using a barbell with a M&M end, and the locking system at the end would work... allowing you to keep your foreskin. I would approach Steelwerks Extreme to see if they can work up a solution.
But just on the PA piercing, I can offer that though I am circumcised, but I turtle up a lot when flaccid. To the point one APP piercer in Montreal thought I was uncut, when I was in to get a different sized curved barbell. It was he who advised getting threaded M&M ends by a company like Anatometal.
The order lead time was long this summer, so I ended up switching to a smaller diameter CBR, and was happy. I'm wearing a 9/16" now which extends the flesh, but just a bit. I was wearing a 1/2" until recently, and even a 7/16" (only while locked up - it a bit thrilling as it was extremely small, but fine in chastity while flaccid). The 1/2" disappears completely except for the vertical flash of metal in the front. It rests higher in my urethra when wrapped up in my flaccid cock.

Perhaps your piercing can be fixed at the end, with an M&M end at the piercing, under your foreskin.
For question two, I can't really advise, as I am cut. The folds of my skin do wrap around my glans when flaccid. I have a PA, and I am using a PA hook in Male Chastity Now's Contender (with a plate instead of rods). My skin moves forward around the hook, but isn't uncomfortable at all. The plate shields the folds of skin from pinching me. No issues with health there. It's actually really comfy.
I'm thinking a somewhat closed device to prevent folds of skin from getting pinched, with a curved barbell with that drain, ending in an M&M, and fixed at the end.
Regardless, you'll have to exercise self control while healing. I healed with 8ga jewelry, and sized up to 4ga. My personal opinion is that 6ga is safe for this application.

I got circumcised as an adult at my wife's request and regret it. The surgeon was unable to remove it all due to constant erections on the operating table. He said that he tried ice but nothing helped. Later on I read that some give the guy female hormone to prevent that so be careful who you choose to do it. There is a whole website about adult circumcisions where similar horror stories are told including lack of sensitivity, scars, inability to get an erection, etc.. Just get yourself informed because no fetish is worth the possible side effects.

Len, Thanks for sharing. I am uncircumcised too and based on similar experiences with swelling have sometimes considered circumcision... but I love my foreskin too much 
That said, I got a PA piercing about 11 months ago and have been working on a foreskin-friendly PA lock concept since then. My first attempt with Lori's tube was a failure due to a poor implementation of the concept on their part. Still need to send the prototype back for fixes.
Has your remaining foreskin returned to a normal sort of state or is it still healing? I'm wondering if you still have swelling side-effects.
Did you discuss your reason for getting circumcised with the surgeon, or did he do the work without knowing?
