It would seem to me that part of the problem is the spacer. I would think urinating would be hard when soft with this spacer, let alone hard. If your penis slot isn't directly over one of those small holes, how can urine come out?
My impression is not, as you said, that holes are not lined up; instead I feel that the shaft is squeezed too much and the urethra is compressed.
So, what to do next?
