Ok. When flaccid lenght is 90-100mm. I did never measure girth when flaccid, however I can enter a 35mm tube with no problems. when erected, length is 140mm and diameter 45mm.

It would seem to me that part of the problem is the spacer. I would think urinating would be hard when soft with this spacer, let alone hard. If your penis slot isn't directly over one of those small holes, how can urine come out?
To be honest, the problem is both with and without the spacer. Maybe without is even worst.
My impression is not, as you said, that holes are not lined up; instead I feel that the shaft is squeezed too much and the urethra is compressed.
So, what to do next?

I know you're looking for numbers to compare so I'll try to help.
I currently use a 90mm long 38mm dia tube and 50mm base ring the top of tube is only a few mm from top of ring and causes the only real pinch point at times. Flaccid I fit snug and comfortable. Uncaged full erection is ~196mm long and 50mm wide and 45mm top to bottom so as a grower vs shower morning wood is painful and purplish until I control thoughts and calm down or splash some cold water on it. Peeing isn't possible right away takes a minute to get room then it's kinda like a kinked hose spraying until the erection attempt subsides. I've broken two CB6000s and trust me the pinches from that break hurt, so I've sworn off plastic. Current CD has several openings that I balloon out when aroused but not painfully like the CB6000. I'm looking at Steelheart 2 as my next one and the way they measure plan to get the 100mm length (since they include the gap and ring material diameter) 38mm dia tube and thinking of the 47mm ring since 50 is a little big most of the time and 45mm is too small and was the only size step in my current one.
The issue you're having with morning wood is similar and I take that as punishment for trying to get erect without permission. But to each his own and everyone's desire and tolerances are different. If it hurts too much maybe bigger would be better but I try to size it for the least erection since that's what it's supposed to prevent and morning wood is only a few minutes of the whole day.
I've had this design, name unknown just a cheap China metal one, for several years and have gone as long as 186 days. Currently stretch began Jan 7 2014 and I don't know when next release will be.
Good luck in sizing, it's a lot of measuring, remeasuring and trial and error.

Thanks, this is really encouraging since your girth is very similar to mine. But on the other side you are having the same pain as me... If you say that it is temporary, I could try. I was worried that the erection did not go away and I removed the cage; I can try to keep it for a while.

Just courious, where did you find a cheap cage with 38mm diameter? I had to ordered a custom one from Steelworxx; generally cheap cages are 35mm diameter.

Just be sure not to get a bulk item order, unless you want 50 or 100. Cheap place to get stuff more than sex toys. I'm sure most are knock-offs but I've had no problems from them. Just check seller reviews.

Really? I have ordered a device from that site, but despite I searched a lot of time, I could not find any 38mm diameter on that site. Do you have a link?
