My best advice is for you (the guy) to relax and don't worry about pissing lol because that's what it feels like, according to my hubby. Also she needs to be patient, it can take some time, especially if you aren't relaxing! :) We wrote about it on our blog too but not an actual tutorial lol just our experience with it.

I have an njoy pure wand and milk myself. I do pee before, but otherwise it really does not matter because I just straddle the toilet while standing and milk myself right over the toilet.
