One of the issues with 24/7 CD wearing, at least for me, is choosing underwear that minimizes discomfort. I have noticed that others also have this issue. I normally wear Ex Officio briefs. They have been very comfortable for me, but with the cage on, I feel some unpleasant sensations and find myself needing to "adjust" fairly often. This generally occurs at work where I sit at a desk most of the day.
After some research and testing, here is what I learned:
Boxers make things worse for me. Too much opportunity for my teeny weenie to get "twisted" or scraped.
My briefs seem to "bind" and hurt all sorts of genital laces

I won't go into why I happened to discover this, but to my delight these briefs look perfectly normal. Under clothes everything is undetectable. Of course, they don't have a fly. That's ok with me since peeing standing up is just not working out well with the CD on. I just thought I would share this very pleasant discovery with you.
FWIW, about a year ago I discovered 2xist brand and they're by far my favorites. Good room in the front, and good support. I found them in Target recently (under the "Evolve" brand) too - though the fabric seems different than the original ones I got (which were 3x as expensive as the target ones.) But the cut and shape of them are still the same.
I do not wear my CD outside of my house but when I did, I found women's panties to work the best for me. The bikini type held me tight and reduced the bulge in my pants. The high cut ones were roomy and comfortable for those times when I did not have to worry about a bulge. Surprisingly, I am not into cross dressing. Just practical.
When locked up, I wear my CD everywhere. I find panties to work well, also. But, finding the right size is critical: too snug and they contort the CD painfully. Too loose and the lock clacks.
(I also recently discovered that with the right pants I can even pee standing up.)
When I am locked, I prefer to wear my standard jock straps ("athletic supporters"). They provide a good amount of support for the additional weight and hold my bits in to minimize the additional bulge. Plus, they are pretty easy to slide aside for peeing. I normally wear boxers, but they really suck for lockup times.
Hi, I find that a thong works very well. Have some regular types and also some custom made by SWIC that have the right size of pouch and thick spandex. Perfect for all activity. Allows the GF to probe/insert a prostate massage device which sure gets the pre cum juices flowing.:0)
I agreed with the others about the balance between a snug fit and having enough room. My g/f sometimes likes to instruct me to wear panties. She bought me 10 different styles to help me find what works best for me in or out of lockup, and what she found most attractive on me.
