A good suggestion - after you get your CB on, pull back all excess scrotum skin so that what is hanging on the outside of the ring is tight, not loose. There should only be enough scrotum hanging on the outside of the ring to hold the testicles.
so that's what I've been doing. Just recently, though, I decided to try the exact opposite: pulling all of the 'slack' through the ring, so that the balls 'hang' in looser skin instead of being tautly held. So far, it seems more comfortable and less prone to pinching.
Balls looser. For sleeping, I get the ring as tight to the body as possible, then use a finger on the very bottom to pull a small amount back to create some slack...otherwise nocturnal erections cause a painful 'stretch/pull' on the skin of the perineum.
Balls tight doesn't work for me- pressure from clothing/movement is very uncomfortable during the day, and worse from nocturnal erections.

I usually use the balls loose arrangement. Sometimes if I am experiencing a little discomfort I go with the balls tight for a while during the day.
I do like the winter though when they try to pull up closer on their own. I find this to be the most comfortable time of the year. Loose and saggy in the summer is less comfortable. Pulling the scrotum tight doesn't really replicate this for me though.

During nocturnal erections my cage, along with the base ring is pulled outside, against the body
Then I wake up, sit for few minutes, let the erection subside & then pull all of the 'slack' through the ring, so that the balls 'hang' in loose & it stops pinching on my scrotum
