You do size to the smallest state. Honestly when I started looking into it and trying rings, I thought there is no way that little ring is going to go over my glans. It does. You have to pull the glans through. I basically stick the tip through then pull while rocking side to side until it pops on. It comes off incredibly easy if you pull the top of the ring forward. Even when fully erect, I can still pop the ring off if I pull int the correct direction.
I was also concerned about the size difference. I am a text book grower. Flaccid I am less than 50mm long and not much bigger around then a finger. When erect I'm just over 15 cm in length and about 39 mm in diameter. I come close to tripling in length and doubling in girth. I am currently using a glans ring sized at 25mm diameter and haven't had any issues.

Sizing is the biggest issue I see. You want the ring small enough that it is snug, but not so small it causes pain. My plan is to include multiple sizes of rings so a person can try a little bigger and a little smaller to find the "goldielocks" fit.

Sizing is important. I am a sample size of one at the minute, but it is very secure on me. I cannot pull it off and I've worn this cage for over a weekend without it coming off. This includes quite a bit of movement and cold temperatures.

I have a couple of active threads over on reddit, but thought I would come share here as well.
I've had multiple people ask for an update, so I thought I would share a bit more. I've refined the design and I feel confident in its staying power. I have worn one for over a week without issue. I've used a force meter and 4.5 pounds of force wouldn't pull it off. I stopped there because anything more was beyond the amount I was comfortable using. I printed the latest one in a metallic blue which looks pretty nice. If you have comments on the design, please feel free to chime in.
I've had multiple people inquire about purchasing. I am making progress on that front. I'm planning to do a beta release in March. I hope to have 10-20 people sign up for the beta. The price will be $109 and you will get a custom device an multiple size glans rings. For the beta, I'll be producing the items myself in ABS. This will give flexibility with feedback and replacement parts if needed. I'm hoping to get feedback on overall quality, fit and value. Assuming the beta goes okay, I expect market price would be in the $150-160 range
Nice - at that price point I'm definitely in for the beta.

One question I have after seeing the pictures (love the color, btw) is about the hook/ring setup. My first reaction is to think that the hook, where it wraps around the ring, will press into/aggravate the frenulum. I assume that's not a problem you've had in testing, but is there a reason the ring and hook are separate pieces?

I haven't had any issues in testing. They are 2 separate pieces because the ring needs to be fairly free floating so it can pivot. I played with embedding it into the shaft, but haven't gotten a working model I was happy with. I don't think there has been any aggravation because the glans is held up and away from the mount. It is held near the center of the shaft. I've also added supports to try and keep everything held away from the moving parts.
