I've never tried to use one with PIV.

The fundamental problem is that to be secure when flaccid, they are too tight to maintain a really hard erection. In my case the erection keeps on fluctuating between 1 o'clock and 3 o'clock.
Still hard enough to be frustratingly stimulated.

As to how uncomfortable it is to cum, depends on the plug. The last one pictured is an exact custom fit, so cum only oozes by, stretching me very uncomfortably. No matter how desperate I would rather not cum wearing it, but some times I have no choice.
