what you are describing sounds like what most of us refer to as ball burn even though it's not really the balls. In my experience and observation the biggest contributor to "ball burn" is too small of a gap between the base ring and the cage. The actual size of either isn't really the issue (although those can cause other issues). If the gap is too small you will get ball burn and there isn't much you can do about it.

Even with the perfect gap you can and likely will get ball burn sometimes. For many years, probably 10 or more, I used a little Aveeno lotion on the area below the ring before bed. This along with proper gap almost eliminated ball burn for me. I don't need to do that anymore and I rarely get ball burn. I have been wearing the same steelheart for a very long time though, it is practically a part of me now.

Good luck with figuring it out.

try using a q-tip to get petroleum jelly (vaseline) between the ring, and the underside of scrotum, if thats where the pain is.

lotion works too but shorter term - think heavy vaseline lasts longer

Your ball gap is likely too small. Ball gap is the distance between the inside of the base ring and the outside of the cage that goes around the cock.
A base ring that’s too large is also often part of the problem. People tend to overestimate the size they need for their cages and rings.

Herman wrote: Thu May 22, 2025 12:00 pm Although it sounds counter to what you might think, a smaller ring will often times help a lot with 'ball burn'. Try downsizing one size at a time, ensuring that you have good blood flow to your boys. You don't want them to be purple because of a ring that is too small. Also, choose a cage that is a little shorter than your flaccid length. That also helps a bunch with 'ball burn'.

Again, I know it sounds counter to what you think would be the solution, but I had the same problem as you when I started wearing a cage, and when I asked for help, the above is what was suggested. I did as suggested, and it resolved my problem. I now wear a cage 24x7 in comfort.
A few people on here have said "smaller is better" and like Herman says, I think you might be better trying a smaller ring.

My own caged male's troubles ended once he moved to a smaller cage with a smaller ring.

The reason is that a correctly sized ring won't move as you try to get erect. It will stay in one place. You say you always use the biggest ring. I agree with Herman that you should downsize. It doesn't sound obvious but it could help a lot.

If Mr. Pickle was here, he would have some exact and scientific way to check for the best size.
Everything's better with a locked male. Better still with a nude, locked male.

thanks, it makes sense ring movement to cause skin irritation there
so now i have used 50mm and above i guess 45mm will be good enough for me?
it should not be really easy to put on but also not extremely painful and tight right?

I would say give a smaller ring a try.
I started with 45mm but am now wearing a 40mm ring. It's much more comfortable.

You mustn't cut off blood-flow or anything but the ring needs to stay in place and not be pulled forwards as you try to become erect.
You mention how easy it is (or not) to put on and it won't necessarily be easy. Try getting one ball through, then the other, and finally just squash your cock through last of all. Once everything is through the ring you can sort it out before putting the cage on.

Good luck!

I started with a 50mm ring myself. Quickly went down to 45mm and thought there was not way I could ever go smaller.
I’m in a 40mm now and have a custom order on the way with both a 40 and a 38mm ring.
The only problem with such small rings is that it becomes difficult to get my testicles through.
