I settled on the KSD-G3 awhile ago as the best compromise to reduce the chance of pull-out, keep things aligned and tight in devises I could/would wear long term. It's not perfect but without it, every ball-trap devise I've tried (that is a big number) has been an epic fail in terms of pull-out prevention - even to the point of slipping out without meaning to.
I've used it as designed on the CB-6000 for the last 3-4 years and as I tried to move to metal devises, being able to retro-fit it with a KSD has been a prerequisite. Someone asked about fitting it to other devises and my results have been mixed. I tried very hard to make it work on a Steelheart and it would not - I re-formed, Dremeled and worked the basic design to make mods that would allow it to function long-term but it was simply incompatible with the basic design and shape. Recently, I successfully retrofit it on a Bon4M with very minimal problems and it has been working very well. In fact, after allot of modifications to the cage (see an earlier review) this metal cage (along with the KSD) has supplanted the CB-6000 as my full-time devise - now in for 4 weeks with no issues and what appears to be a long stretch ahead of me.
Couple things to know on the KSD if you're thinking about it or haven't used it for long-term wear. It will create a pressure point on the top of your penis which takes time to adapt to. I found Neosporin applied under the devise helps and keeps any abrasion to a minimum. I find the least restrictive size generally all that's necessary to get adequate security but everyone's different.
A bigger problem for me has been the long term effect KSD if you're keyholder is into longer-term lock-up (over a few weeks). I find that if I've been in with a KSD more than a month or so, when I'm taken out and have an erection, my penis will hurt (badly) at the point where the KSD has made an impression. This pain can remain for weeks after I'm out making erections less fun. Again, I only experienced the impact on long-term lockup but it was very noticeable and I didn't figure where the pain was coming from for awhile. My guess is the the penis gets used to the compression during erections in the cage and when that is going, it creates some kind of stretching. It does go away and I've seen no long-term issues and I haven't heard anyone else report it so perhaps it's just me.. Best.

I found a makeshift cheap and free alternative to try. We're new to the chastity scene and I found I can slip out of the CB6000s if extremely flaccid and have enough lube.
I took an older rubber silicone cock ring I had. Looped the ends on the outer pins, and UNDER the middle locking pin. It pushes the penis down a bit and it hasn't come out since. The penis also stays further inside the cage.

I bought these and don't know why, or for a better set of words, I shouldn't have bought it at all. First of all, my cock is too wide for the CB-6000 as it is; secondly, with this being said, I can barely only use the largest size because none of the smaller sizes even comes close to fitting.
I wish I could post a picture of the damage without showing the whole cock and being kicked off. Bare with me as I try to explain it, as I try my best with only words. I must admit, it works well, maybe a little too well. I like that it keeps my cock pushed up to the top, preventing slippage and allowing me to urinate more easily. What I don't like, is the fact that it digs in were the bottom tip of it meets the top side of your cock. It really digs in, causing skin breakage and minor bleeding. You can definitely see the damage when you take it off. I contacted Kept For Her, and they gave me instructions on how to fit it better (for those of you wishing to know more about the how to, I'll post the instructions below). That didn't work either, it just made it loose on the back end, where it attaches to the locking pin.
What I would like to know is how to wear it without it cutting in. If there is anybody with concrete instructions please let me know! I would prefer it in, as it does help with not allowing my cock to slip back, but it hurts as well as causing physical damage, which I'm sure is never the intent of any safe playing mistress.
The instructions as given to me, on how to get a better fit, are as follows:
Heat up a cup of water big enough to place the KSD-G3, with another cup handy to hold it down, to a boil in the microwave. Place the KSD-G3 in the hot water, weighing it down with the extra cup. Make sure the cup is in the sink so you don't get scalding water everywhere, with the potential of burning yourself. Hold the second cup in the first cup a little, just to allow the KSD-G3 to be fully submerged for 15 seconds or so to make it malleable. Pull it out (caution: it will be hot), and bend the side wings in or out, or the tongue upwards or downwards to achieve a good fit. Place the KSD-G3 in your cage like you normally would when you wear it, putting the whole thing in the freezer for 5 min. After this is done, check for fit and repeat as necessary.
