Forgive the relatively long post, but I want to provide as many facts and as much background as possible before asking advice.
My wife and I started MC play at the end of September. We started then with a CB6KS. After a bit of fiddling, I found the 1 3/4" ring and the shortest spacer to work well. It's very comfortable, but not quite as secure as I'd like (or at least not as secure as I'd like it to feel). About 90% of the time I fill the tube to the tip, and the remainder of the time Mr. Happy ends up 1/4" to 1/2" shy of the end of the tube.
About a three weeks later we decided to upgrade to steel, so we ordered sizing rings from MM in anticipation of ordering a JailBird. I started with the 1 3/4" base ring. Per MM's instructions, a properly fit ring should allow you to get your index finger in it up to the first knuckle. I could get my index finger in well past the first knuckle, and I could almost get my index and middle finger in up to the first knuckle, but not quite. I then tried the 1 5/8" base ring. It fit exactly as MM described it should, so that's what we decided to order, in its oval form. We were so confident in our measurements that we went ahead and ordered the double ring only, despite admonishments from MM that it couldn't be resized. That was mistake number one.
Moving on to Mr. Happy's new environs, I first tried the 1 1/4" sizing ring. It slipped easily over the head, but once on the shaft it was easily moved around. As I recall the MM instructions, the ring should be snug, but not tight. The 1 1/4" ring was not snug, so we tried the 1 1/8" ring (the smallest one in the sizing ring set). There was a little bit of effort to get him past the head, but once on the shaft, it felt snug, but not tight, so we ordered a 1 1/8" diameter cage.
As for cage length, we took measurements over several nights that typically ranged between 2" and 2 1/4" flaccid. We decided to go with 2" as our final measurement. MM's instructions say to subtract 1/4" from the result. Based on comments we had read here and other places that essentially said "when in doubt, go smaller", we subtracted 1/2" and ordered a 1 1/2" cage. That was mistake number two.
The JB arrived about a week ago. Getting the ring on was a bit of a chore, but that wasn't totally unexpected. Getting the cage on was quite a humorous event. Mr. Happy was behaving like a lobster trying to be put in a pot of boiling water. But, with a bit of lube and a lot of determination, we managed to get the cage locked into place. The poor little guy looked miserable. Bits of him were poking through every available space in the cage, and a fair bit of him was caught between the ring and the cage, taking up valuable real estate needed to accommodate other parts of my anatomy. With the help of some Q-Tips and a little bit of time, I managed to get all of Mr. Happy into the cage. It wasn't comfortable, but neither was the CB6KS at first. I figured it was just a matter of getting used to it, so I rode it out for a few days. That was mistake number three.
After three days, my scrotum, though still pink and warm, was swollen to the size of a softball. I was also feeling intense pain around the circumference of the base ring (no doubt due in part to the swelling). When we took the cage off, Mr. Happy was grossly contorted, with several abrasions presumably caused by skin poking through the bars of the cage and rubbing against fabric. Due to the swelling in my scrotum, it was several hours before I could get the ring off. Once off, it was apparent I had a serious abrasion around the entire circumference where the ring had been. It can best be described as looking like a rope burn.
So now to point of this rambling post. Before we plunk down another $400 or so for a new JB, we'd like to get some input as to where we went wrong. We haven't measured the JB to see if MM made a mistake, but we're assuming for now they didn't.
It's apparent that we need to order a longer cage, but how much longer? As I recall, the tube on the CB6KS is 2 1/2" long, and I fill it most of the time. This fact is at odds with my measured length. I suppose sitting naked in an air conditioned house so we can sneak up on Mr. Happy quickly vs. being slightly aroused in the confines of a tube could account for some of the difference. I would imagine that it's also true that the ring of any device is going to have the same effect as a cock ring, making the penis a little bigger that it would be otherwise. Should future measurements be made while wearing the sizing ring for base ring? I seem to recall reading somewhere that MM can make a long cage shorter, but can't make a short cage longer. Is that true? If so, I think I'd tend to order on the long side and get it adjusted later if necessary.
Then there is the cage diameter. Again, as I recall, the CB6KS is a 1 3/8" diameter oval, a full 1/4" inch larger than the diameter of the JB we bought. That's a lot of extra volume. I find the pressure I feel as I attempt to get erect inside the tube pleasurably frustrating (to say the least). My wife seems to take great delight in this as well. I believe the smaller diameter of the JB will work well towards that objective, but we're concerned about the abrasions I experienced in the current 1 1/2" cage. Would a longer cage help alleviate or eliminate this problem? Any comments on our determination that a 1 1/8" diameter cage was appropriate?
As someone on this forum pointed out somewhere, the length and diameter of a flaccid penis are not independent variables. Decreased length equates to greater diameter and vice versa. How does one go about factoring this phenomena into the sizing decision?
Finally, there's the base ring. We're really not sure what went wrong here. It seemed to fit the MM specs perfectly, and I like the feeling of added security that it provides relative to the 1 3/4" CB6KS ring. Could the swelling I experienced have been caused by the cage being too short? I know a round ring will have a larger area than an oval of the same circumference, and the sizing ring is round, not oval, so we're left to wonder if it would have worked out better if it were round. Or does it sound like it was just too small?

As I read your post, I noticed how similar my measurements are to yours. I have the 1-3/4 base ring on my Jailbird. At times I thought it might be a little big. Since we are probably going to order a Watchful Mistress, I thought maybe I could go one size smaller with the base ring. I tried wearing the 1-5/8 sizing ring last week during my chastity vacation. It went on and seemed OK at first. But after half a day I noticed things starting to swell, so I took it off. I decided that 1-3/4 is my correct base ring size.
As for cage length, I subtracted 3/4 inch off my normal length. It is perfect. That is because till you allow for the thickness of the base ring, plus the gap, plus a bit of slippage, I am right there at the end all the time. It sounds to me like your cage diameter should be 1-1/4. That is what I went with, and it fits like a glove most of the time. Yes, I can fit the 1-1/8 sizing ring on if I try. But the 1-1/4 diameter is comfortable with very little room to grow.

1). Should the cage rings be the same as the diameter of my shaft, or cockhead?....and Mistress MM said the cockhead diameter is the one to use. I did so, and have a small amount of space between the bars and the shaft....which is normal.
2). Where do I measure length...the top or bottom of my penis? Her answer was the bottom...from the tip to where it meets my scrotum. I was most worried about this measurement because the point of where the shaft meets the ballsac is somewhat subjective....it depends upon if my ballsac is hanging lose, or uptight. So I just sort of 'guestimated' where to place that end of the ruler, taking care not to push it into the scrotum and distort flaccid length.
I took length measurements many times over three days, finally arriving at a 'smallest' length of 2.5 inches....which is a far cry from my earliest measurements, which were considerably longer because Beth kept 'helping' me. Sorry to hear you are experiencing problems with your new MM JB. Sucks when you spend that kind of money and endure the wait only to find it's not working.
I too wore the CB6000s for a little over a year. Once acclimated and relaxed to the feeling I sat, as you, about 1/2" to 1/4" from the tip 90% of the time. I also filled the CB6000s to the sides. Not real difficult to get in it but touched the sides almost all the time.
This is what I ended up ordering from MM:
Base Ring - 1 3/4"
Gap - 3/8"
Cage Diameter - 1 3/8"
Cage Length - 2"
I found the base ring to be a bit too tight so I kept the 1 3/4" and ordered a 1 7/8" ring. After the breaking in period I found that I was once again sitting about 1/4" to 1/2" from the front bars 90% of the time. The cage diameter was perfect but over time I found the base ring to be a bit too large. I ended up keeping the 1 7/8" base ring and had MM add on another to make it a double but wanted something between that and the 1 3/4". They obliged and added 1/16” to the 1 3/4" ring making it 46mm.
When I ordered my Watchful Mistress earlier this year I kept the cage diameter and gap the same but ordered a 1 3/4" (44.5mm) ring and shortened the length to 1 3/4". I now find I don't ever wear the double ring any more and switch between the 46mm and 44.5mm base rings. All my rings are oval also.
I enjoy a really snug fit and prefer to catch the little bugger before it has a chance to grow. It's also very easy to fit into everything I wear, which is pretty much jeans this time of year.
Never experienced the "softball" you described but I always apply lotion twice a day around the base ring. Once after morning shower and again at night before bed. Without it I would be miserable down there.
I do know that MM can shorten a cage because I had my JB modified but I'm not sure about adding to it.
Don't believe I have an answer for your problems but I thought I could chime in with what has worked for me since it sounds like we are about the same size.

Just so you know, you're not the only one to go too small the first time. My first MM cage was 1 3/4 in long and 1 1/8th diameter. I wanted it to be snug but I over did it. Ended up selling the device and buying another MM device with the cage 2 in long and 1 3/8th diameter and I also fill it most of the time. If you put it up for sale you will probably lose about $100 overall. They resell very well. Very few are lucky enough to get it right the first time so don't be discouraged.

I agree that you shouldn't feel totally bummed, you may be able to find a buyer for your device and recoup some of your money. MM devices do seem to resell well.
It's been said here many times that when you order a MM device don't be afraid of asking them questions.
When you were unsure about the shaft diameter you should have Emailed MM and asked about it. I had the same problem Regular Joe had, with the same result;

If you decide to try another MM device a recommendation on the base ring, especially with you just starting at the end of September. When you order a device order two base rings. One that is almost perfect, but slightly large, and the next size smaller. The reason for this is that almost everyone wants a smaller base ring as time (think many months) goes on. Your body adjusts and a smaller ring fits better. I would also suggest a single ring if ordering a double.
Have you measured the base ring to verify it's what you ordered? I had a problem once with the wrong size, when I told MM about it they fixed it immediately.
One last thing to ask. Have you contacted MM with your problem? They may have suggestions that may help with your next device, wherever you get it.

A few things I've heard about the double rings. When you go with the double rings you need the base ring to be a little larger, especially it you're on the high and tight side. The double rings will also make the device stay closer to the body, which may change the measurement. I can totally relate to this. When I first rec'd my CB3k, I found that the smallest ring was the only one from which it was impossible to escape. I put that on and Mrs. Twisted took the key...and so began what turned from erotic torment to sheer torture.I was also feeling intense pain around the circumference of the base ring...Once off, it was apparent I had a serious abrasion around the entire circumference where the ring had been. Things were fine during the day when I was awake and had some control of [attempted] erections, but nighttime was a different story. Not being able to exert any mental control, nocturnal erections would proceed to the point where I would awaken in intense pain, which got worse as the days and nights went on.
Eventually, I was in pain constantly. I tried to 'tough it out' but after the third night I finally had to get it off, I couldn't take it any more. I was severely abraded as you describe, and the musculature was deeply bruised as well. It took quite some time to heal.
So, the next attempt was with the next to largest ring, which, though escapable, otherwise worked OK. Over the next 12-18 months the size was gradually reduced by wearing the next smaller ring for a short period, then going back to the larger size, then smaller for a longer period etc. Eventually, I was in the next to smallest ring, which has been made slightly smaller by the addition of epoxy (used to mend the broken hinge).
I could probably tolerate the smallest ring now but have been hesitant to try it because of the hinge rivet, which I've discovered can be quite painful whether it is turned forward or back, and I'm not sure I'd be able to get it on without being hinged...especially if the thickness were increased by using epoxy to cover the rivet.
In short- while the smaller size may be the correct size for you, it could take you quite a bit of time to adjust to wearing it. You may need an extended period of acclimatization by going from a larger size to the (a) smaller size.

As others have said, contact MM to see what they advise. They are good folks. I am very happy with my JB. Obviously I got the sizing just right. It is very snug but I've had no problems at all. I am presently at 37 days without release headed for 180.
One suggestion. Several years ago before my wife and I got serious about this we played around a little with cock rings etc. If you have a Tractor Supply close make a visit. In the logging chain area they have an assortment of brass rings of various sizes. Not really off the wall sizes but the standard.
1 1/2, 1 3/4, 2. Since you think the 1 5/8 is too small, get a couple of 1 3/4 and glue them together. Voila a double ring. Wear it for some time checking often and see if the size works for you. By the way, they're cheap. About $2.00 each. They have smaller ones too that you could use as a cock ring.

Good advice, though I'd be very careful placing too small a metal ring around your shaft...it is quite possible to have so much constriction that the blood won't leave your erect cock, and it will be a trip to the emergency room to have the ring cut off. It happens; they've seen this stuff before and know what to do. Finally, the most common medically recommended maximum time period to wear a truly constricting cockring is 30 minutes. That critter needs fresh blood for oxygen to keep the old boner meat alive!

I'm not a gynecologist, but I'll take a very close look.

Definitely be careful if using it as a cock ring. I have but make sure I can get it off even if I'm still erect. Use one just a little smaller than your cage diameter and you shouldn't have any problems. At least I never have. Of course I'm not getting any erections for quite a while so the subject is moot for me at this time.
