One use lots of lotion and use it frequently. second just wear the ring without the cage. start with the largest ring and keep it lubricated with lotion (not KY or Vaseline)....

Been wearing the biggest ring and nearly the longest pin, but my testicle inadvertently almost slipped through the ring and I could not move for it was stuck. Sooooo, do I go with the next shortest pin or next smaller ring?

Yes, lotion is the key to the device (yes pun intended). What I found what worked over the long haul is try the smallest ring comfortably and the longest pin. then decrease ring size over time.
